Your GP or specialist completes the Fitness to Drive form and confirms that an assessment is required.

Your GP sends us the completed Fitness to Drive form directly, or gives you a copy to forward to us.

We review your medical history, vision, physical ability, cognitive skills, and road-rule knowledge.

You complete a practical test in a dual-controlled car with the OT and Driving Instructor.

We discuss your results and provide a written report outlining our recommendations.
